About the Role
Our client, a growing 143-bed acute care hospital located in western North Carolina, is seeking a Director of Case Management to lead and oversee all aspects of Case Management operations. This role plays a critical part in improving care delivery outcomes across the hospital, managing resource utilization, and ensuring effective discharge planning and patient advocacy.
The ideal candidate will bring a strong clinical background in nursing, extensive experience in care coordination, and proven leadership in hospital settings. This is a high-impact role responsible for aligning case management practices with regulatory standards and the organization’s broader strategic goals.
Core Responsibilities:

Lead and manage the Case Management department, overseeing discharge planning, utilization review, and social work functions.

Drive performance improvement initiatives to reduce Length of Stay (LOS) and enhance overall clinical services.

Work collaboratively with executive leadership, physicians, and department managers to implement evidence-based best practices in care management.

Ensure adherence to CMS, Joint Commission, and other relevant regulatory requirements.

Improve and streamline the utilization review process, ensuring smooth patient transitions across the care continuum.

Participate in strategic planning and contribute to system-wide performance enhancement initiatives.

Mentor and educate staff and physicians on case management standards, payer requirements, and industry best practices.

Analyze care management trends and recommend initiatives to drive cost savings and enhance revenue opportunities.
Qualifications:
Required:
-
Associate’s Degree in Nursing.

Active RN license in North Carolina or a compact state.
-
Current BLS Certification.
-
CCM Certification (or the ability to obtain it within the first year of employment).

At least 3 years of clinical experience in acute care or care coordination.

Comprehensive understanding of Medicare/Medicaid admission and review requirements.

Strong leadership, communication, and decision-making abilities in high-pressure environments.
Preferred:

Bachelor’s or Master’s Degree in Nursing or a related field.

3+ years of healthcare management experience.

ACM/ACM-SW certification preferred.

Familiarity with InterQual criteria and government billing regulations.
Interview Process:

Step 1: Virtual interview with the CFO and divisional leadership.

Step 2: On-site panel interview with facility executives, department heads, and potential direct reports.
